Community Support Services |

Te Puna recognises that health and wellbeing are affected by many things; some will be physical, others mental, some might be spiritual, while yet others may be whanau related. Our environment can affect these areas greatly. Our Community Support Services are offered to awhi (support) people who are troubled by issues that are affecting these things. We will meet with you and discuss/assess what areas you feel that support might be helpful. |

If you are registered with Te Puna PHO you are entitled to access our community services at no cost. |

Our Community Support Services are based in Northcote but we are highly mobile and currently have clients from Devonport to Whangaparoa. We even have two members of our mental health team based in Wellsford! |

How we work... |
Te Puna operates a 'wrap-around' service model. This means we provide services to support every aspect of an individual/whanau health and wellbeing. It also means that the client is the driver of the process - you call the shots! Our role is to use our collective expertise to guide and support you to reach your goals. We call this model the Te Puna IMAP model and you can find more information here. |

The Community Support team |
| Based in Northcote, although also available at our other sites, the Community Support team provides a system of support and care for the community. |

| Community Support Workers |
Our Community Support Workers (CSWs) support whanau/clients in crisis, along with support and liason from other services in the community. |

| Iwi & Whanau Mental Health services |
The Iwi & Whanau Mental Health services provides mobile services for adults/whanau, who have mental health issues. More information here. |

| Asian Community Support Services |
Te Puna PHO funds an Community Support Worker to worker alongside the Asian community. While much of this CSWs work is based at Windsor Medical Centre, this role creates a vital link between the Asian (Chinese in particular) community, and the services available within Te Puna Hauora. |

| Parenting Programme |
Our Parenting Programme is here to help whanau and parents gain the necessary skills to provide better quality care for themselves and thier children. More information here. |

| Youth Worker |
The Youth Development Plan Service comprises of a Maori Youth worker who works with young Maori at risk from the Judicial system. |

| Anger Management Services |
| Te Puna provides Anger Management services for Maori men who need support dealing with their emotions and behaviours. |
| |
| Counselling for Maori Women |
| The Manawahine Taumatanga trust with Rangi Davis delivers Wananga and counselling from a wairua persective for Maori women living on the North Shore. |
